Young Chelsea Foundation coach Tom Horrigan has been given the honour of presenting the Premier League trophy to Gary Cahill and John Terry on Sunday.
Blues legend Dennis Wise will kick off the celebrations at Stamford Bridge by walking the trophy out, before Horrigan takes over for the final leg.
The club nominated the 21-year-old coach as the Premier League Kicks Hero in recognition of his involvement with the social inclusion scheme.
Cahill and Terry will lift the trophy together following Chelsea's final game of the season against Sunderland, with the latter saying his goodbyes at Stamford Bridge.
