With United already up 2-1 in the League Cup match, Ander Herrera booted a ball long up the field. Rather than come out of his box to deal with the danger, Smith inexplicably stayed on his line and then compounded his error by whiffing his attempted clearance.
The result was the easiest goal Marcus Rashford will likely ever score. United would go on to win the match 3-1, snapping a three-game losing streak.
