Danny Ward was Huddersfield's hero, as he saved Jordan Obita's penalty allowing Christopher Schindler to send the Terriers into the Premier League.
Izzy Brown had the best chance to win the game in normal time, firing wide from barely a yard out with the goal gaping.
But after that early opportunity neither side did enough to win the game, with only three shots on target in 120 minutes, meaning it came down to a shoot-out, where Huddersfield triumphed.
