Dennis Wise will kick-off Chelsea’s title celebrations when he takes the Premier League trophy out onto the Stamford Bridge pitch on Sunday.
Antonio Conte’s side will be crowned champions after the final game of the season against Sunderland on Sunday.
Former Chelsea skipper Wise will walk out with the trophy ahead of the post-match festivities.
Poignantly, Wise was club captain when current skipper John Terry broke into the first team in the late 1990s.
Terry - who will leave Chelsea at the end of the season - will lift the trophy with vice-captain Gary Cahill at Sunday’s ceremony.
