John Terry received a guard of honour as he was substituted in the 26th minute in his final Chelsea game.
The club legend wears the number 26 and was replaced by Gary Cahill as Stamford Bridge rose in unison to applaud their departing hero.
Terry applauded the crowd and took in the adulation for a final time as he walked off with the score at 1-1.
After 22 years of service, five league titles, five FA Cups and a Champions League, Terry offered the fans a heartfelt goodbye en route to the ground before the game.
