According to the Yankees PR Department, the Bronx Bombers have let go of promising young infielder Yangervis Solarte as part of the exchange:

The New York Yankees have been targeting Headley for years now and finally came to an agreement with San Diego that was tenable for both clubs going forward.
Headley's offensive numbers have taken a dip in 2014, as he is hitting just .229 in 307 at-bats with seven home runs and 32 RBIs. However, his glove work remains top-notch and is past track record of success at the plate was enough for Yankees general manager Brian Cashman to take a chance on the 30-year-old.
Headley is in the middle of a one-year, $10.25 million contract and will hit unrestricted free agency this winter. The Yankees will likely target a new deal with Headley this winter, perhaps in hopes of obtaining a discount based on Headley's struggles of late.
