The Uruguayan joined Middlesbrough from Southampton on a free transfer, but could do very little to prevent the club from suffering relegation from the Premier League last season.
And Ramirez could be on the move again this summer, with Serie A side Sampdoria interested in snapping up the 26-year-old.
According to Sky Italia, the Italians are closing in on a deal for Ramirez and are hopeful of completing the transfer as soon as possible.
With Middlesbrough set for Championship football again next season, Ramirez may yet be playing in the Italian top flight as Sampdoria look set to complete the deal.
