The Brazilian thew himself at Bruno Fernandes free kick in the 89th minute, with his near post header beating Forest goalkeeper Matt Turner.
Casemiro's effort takes Man United into a record 48th FA Cup quarter-final.
Ten Hag's side will face the winners of the match between Liverpool and Southampton.
