Walter Mazzarri’s Watford and Tony Pulis' West Brom were the biggest casualties of round two of the EFL Cup.
The Hornets lost 2-1 to League One Gillingham at Vicarage Road. The Gills were hammered 5-0 by Scunthorpe at the weekend but they bounced back to take the Hornets to extra-time as Bradley Dack's goal gave Justin Edinburgh's side the shock victory.
Meanwhile the Baggies were beaten by Rob Page's Northampton, also of League One, on penalties following a 2-2 draw.
And elsewhere, Derby and Carlisle took part in a 27-penalty shootout, with Nigel Pearson’s Rams coming out on top (14-13) at the iPro following a 1-1 draw.
