Actually, Duke and UNC did play four times in 1978-79, in the old Big Four tournament (Duke, UNC, NC State and Wake Forest had a Tobacco Road tussle in the ‘70s).
The closest they ever came in the NCAA tournament was in 1991, when Duke legendarily upset UNLV and UNC lost to Kansas.
Good thing it didn’t happen: when the crowd went in, the Duke fans went in one door at the Hoosier Dome and, just down the way, UNC fans entered via another.
